are you definately setting the lines whilst the shifter is in 4th? If so there must be something else snagging or obstructing the cables route. Check the outer cable end at the wheel end, you will see a black arm sticking out of the bottom of the hub which holds the outer cable end (ferrule). If the cable has been pulled tight this could have popped out. If you try to reset the lines with the cable incorrectly seated it will be near impossible to adjust. This is what I would check first.
Next move down to the very end of the cable, you will see a 9mm pinch bolt. Follow the cabel out of the pinch bolt, all the way back to the outer cable end. It should run in a pre-cut groove in the plastic gear selector on the hub. Again if it has become snagged it could have popped out of this groove, casuing abnormal adjustments.
Thirdly check the enitire cable outer to see if there are any kinks or breaks in the cable. Sometimes the outer itself can push through the cap on the end (ferrule) and the insides become the outsides

. Again this results in abnormal adjustments.
I would put money on it being a cable routing problem somewhere as the shifters and hubs are pre-adjusted and can't really be messed around with. The cable is the only variable.
Has anything happened prior to this? Has the bike been loaded in a car or had the wheel off? Has it been crashed or dropped? Bars loosened or turned? Any info would help me pinpoint the area where the problem lies.
