cwah, you turn the spanner in the direction you pedal to remove the cups. If you hammer it like in the picture, you are tightening it. The chain side cup (fixed side) has anticlockwise thread, the non drive cup (adjuster side) has the normal thread.

you are small, you need a lightweight bike and with smaller wheels, 24 inch or 20 inch. Not a heavy bike with large wheels.

AFAIK, Cyclotricity do not do BPM rear hub kits. You need a lot of torque to move 200kgs up hills.
